Intake forms will be sent to you to complete through Junovo once you have requested an appointment and completed payment. These forms gather essential information about your medical history, current health status, symptoms, and lifestyle factors.
Once your completed intake forms is received and reviewed and payment is completed, you will receive your blood requisitions to complete.
After completing the intake forms and payment you will receive blood work requisitions. This step is crucial as it provides objective data on your current hormone levels, along with other relevant health markers. These tests help to identify any deficiencies or imbalances that may be contributing to your symptoms or health concerns. It is suggested to complete blood work up at your earliest so it is received in time for your first consultation.
In our first meeting, we’ll review your medical history and current medications.

The Nurse Practitioner will have fully reviewed your intake forms, symptoms chart and blood work prior to your consultation.
